What is PICSI

PICSI, or Physiological Intracytoplasmic Sperm Injection, is an advanced fertility treatment designed to improve IVF success by selecting healthier and more mature sperm. This technique closely mimics the natural fertilization process by allowing sperm to bind to hyaluronan, a substance naturally found around the human egg in the female reproductive system.

Unlike standard ICSI, where sperm selection is mainly based on appearance and movement under a microscope, PICSI focuses on the biological maturity and DNA integrity of the sperm. Only sperm capable of binding to hyaluronan are chosen, increasing the chances of successful fertilization and healthy embryo development.

PICSI

Principle of the PICSI Method

  • Hyaluronic acid is a natural component of the egg’s outer layer.
  • Only mature sperm have receptors that allow them to bind to hyaluronic acid.
  • This binding process occurs naturally during fertilization.
  • In PICSI, sperm are placed in a hyaluronic acid–coated dish.
  • abnormalities.
  • Sperm that successfully bind are selected for fertilization.
  • The selected sperm are injected into the egg using the ICSI technique.
  • Hyaluronic acid–bound sperm show better morphology, stronger DNA integrity, and fewer chromosomal

When IVF doesn’t succeed, what are the next steps?

PICSI is recommended for couples who need advanced sperm selection to improve fertilization outcomes, including

High sperm DNA fragmentation (high DFI)

How Is PICSI Performed?

A special dish coated with hyaluronan—similar to the natural layer surrounding the egg—is prepared. Ready-to-use PICSI dishes are commonly used in modern laboratories.

A semen sample is collected from the male partner and processed to separate healthy sperm from other components.

The prepared sperm sample is placed on the hyaluronan-coated dish. Only mature and functionally healthy sperm bind to the surface.

Under a microscope, the sperm attached to the hyaluronan are identified as high-quality sperm.

The selected sperm are then individually injected into the egg using the ICSI technique to achieve fertilization.

How Is PICSI Performed?

  • Poor overall sperm quality
  • Low sperm motility (reduced movement)
  • Abnormal sperm shape or structure (morphology issues)
  • High sperm DNA fragmentation levels
  • Previous IVF or ICSI failures linked to male factor infertility
  • Need for improved fertilization and embryo development outcomes

How Does PICSI Influence IVF Success Rates?

PICSI plays an important role in improving IVF outcomes by enhancing sperm selection and embryo quality. Clinical evidence shows higher pregnancy rates and a reduced risk of miscarriage in IVF cycles using PICSI. By choosing sperm with intact DNA and strong fertilizing ability, PICSI increases the chances of successful assisted reproductive treatment.

FAQs related to PICSI

Research and clinical findings show that PICSI can enhance embryo quality and improve pregnancy outcomes by selecting sperm with higher maturity and better genetic stability, particularly in male infertility cases.

Yes, PICSI is considered a safe and reliable laboratory procedure. It follows the same steps as ICSI, with an additional sperm selection method, and does not pose extra risk to the egg or embryo.

  • IVF: Fertilization occurs naturally by placing eggs and sperm together in a lab dish.
  • ICSI: One sperm is injected directly into the egg.
  • PICSI: An advanced version of ICSI where sperm are first selected based on natural binding ability, indicating better quality.

The cost of PICSI depends on individual treatment plans and whether it is used alongside IVF or ICSI. At AMVI Hospital, detailed cost information is shared during the fertility consultation.

Yes, PICSI is helpful in cases with high DNA fragmentation as it allows embryologists to choose sperm with intact genetic material.

The sperm selection for PICSI is completed on the same day as egg retrieval and fertilization and does not significantly extend the treatment timeline.

PICSI does not introduce additional risks beyond those associated with standard ICSI and is safe when performed by trained fertility specialists.

Yes, PICSI is often combined with IVF or ICSI and can be part of a comprehensive fertility treatment plan based on medical advice.

Healthy habits such as balanced nutrition, regular exercise, avoiding tobacco and alcohol, managing stress, and following medical guidance can positively influence treatment success.

You can consult AMVI Fertility & Maternity Care at our Hyderabad locations in Attapur and Puppalaguda, where experienced fertility specialists provide compassionate, advanced care for PICSI failure and infertility.

After PICSI, embryos are monitored for growth and development. Embryo transfer and pregnancy testing are planned, followed by ongoing care and follow-up at AMVI Hospital.
